Patient and Family Advisory Council

In an effort to continue improving its quality of care, Norton County Hospital has created a Patient and Family Advisory Council (PFAC). On the council will be community advisors who will be asked to serve alongside health care staff to ensure multiple perspectives and experiences are both heard and integrated into improving quality of care. To Apply

Health care professionals within Norton County Hospital, as well as patients and family members of patients, are invited to apply at any time. Access more information about applying below.

Application:

Applications can also be obtained at any admissions desk at the Norton County Hospital and Norton Medical Clinic. Following the application process, applicants will be interviewed by the PFAC Committee who will make the final decision.

The application includes directions for submission. In addition, we have included below the contact information for application submission and to get any of your questions answered. Applications can be submitted in person or emailed.
